None of your companions can die if they are following you, but they can die if they aren't. An example would be if you are in combat and you tell her to part ways and she gets killed by the person or creature attacking you because she stopped following you. I used "she" because I am referring to Lydia, your bodyguard you get from Whiterun for being the Thane.

To obtain the Skull of Corruption in Oblivion, you must complete the "Symbols of Office" quest given by Shivering Isles priest Dervenin. In the quest, you need to retrieve the skull as a key item. Once you have completed the quest, the skull will be given to you as a reward.
Almost all blacksmiths sell them. Also you can smelt them yourself if you have some iron ore and corundum ore. Sometimes when you go into random people's houses to get them. There should be safes or strong boxes in various rooms. If you can lockpick them, chances are that there will be goodies in there...maybe even a steel ignot. Other times you can go into tombs or any underground burial place and kill some draugrs. They MIGHT have ingots with them, but most times they dont. The only place I know with a draugr carrying an ingot is in the quest The Golden Claw.
