The first thing to do is call your eye doctor and ask them who accepts this in your area. Or , you can call the lyons club in your area and ask them to send you an application to fill out. First you fill it out and then they have mothly meeting and go over the applications. They will then contact you once the application is approved or gets a deny. Hope this helps you !
