Even though many would think in the first book that it's definitely Seneca Crane, the head Gamemaker of the 74th hunger games, I believe that throughout the first two books it is President Snow, and in the finale book a mixture of Snow and Coin. But to focus on the first book, what Seneca did was honorable even though it did cost him his life (for those of you who don't remember he was the one who let both Peeta and Katniss live). Snow on the other hand saw this as an act of betrayal, that it was a crime and he broke the only rule of The Hunger Games, only one tribute must live. He then thought that his crime was so big that he must be euthanized for that reason.

Yes, but not for members with Bad Conduct/Dishonorable. Members with and OTH (Other Than Honorable Discharges) are still eligible and can receive a burial flag. There are no laws that prevent someone from purchasing and draping a casket with a flag however one will not be provided military honors by the guard to fold and present it to the next of kin if you have a dishonorable discharge. You must have at least an Honorable or OTH discharge.
If you are talking about in the book, there really is no expectation for the Hunger Games. At the Reaping, it's really anybody's chance and they can't expect you to be ready for anything. I think they (the Capitol) at least expect you to be willing to fight and try to kill other tributes. If your talking about the book, expect a wild, action based story with romance mixed in between.
