EDIT: Although that is what we see and believe without question, the man sitting next to him near the end before the "heart attack" says some things and grins in a rather sinister manner. And as to why he holds ezio's hand and says " kuranjo vecchio" we do not know. It might've been poison, or some other type of subtle murder. That theory is my own of course, but i'd like to believe that it was a heart attack as well.
Ok guys. the person who sat next to ezio was just a random guy his not at templar . he did not posion ezio... he said in english (courage ezio). then leaves because he saw that ezio is not feeling well .even though the random person seems disrespectful this actually reminds ezio about his teenage years . and also he already wrote a letter to his wife because he already know his time is coming.
