The question is irrelevant. The PPACA is budgeted, and is scored by both the CBO (Congressional Budget Office) and the GAO (General Accounting Office). The costs of the PPACA to the federal government are part of the general budget.Of course, with a program of the complexity and size of the PPACA, the future outcomes and costs are unknown, and can only be estimated. Which, again, the CBO and GAO both have done, and are included in all future formal budget projections.

The PPACA makes signficant changes to Medicaid (the program for the poor), but mostly administrative changes to Medicare (the program for the elderly). Medicare is a separately administered program from most of the PPACA system, and the only really significant changes to Medicare that were included in were administrative (covering the method of payments, and amounts), and the eventual elimination of the Part D "donut hole" coverage problem. Is it true that under the ObamaCare bill your primary physician must be the doctor admitting you to the hospital in order for Medicare to pay for it?
The PPACA does NOT apply to any Medicare program or insurance coverage. Medicare is NOT part of the PPACA. Medicare is the old-age social insurance program for those 65 and up. If you mean Medicaid (the health care coverage for poor people), the PPACA does not specify such details. Medicaid is administered by the states, with certain funding and general requirements coming from the Federal government. The details of required procedures to be followed for payment are up to individual states to decide upon. So, in that context, you will have to ask your state's Medicaid program about this issue.
When will ObamaCare start?
Some of the provisions of the PPACA started in 2012. The Exchange system started in 2013, with initial sign ups going through 31 March 2014. Certain groups have petitioned for delay waivers, and have been given more time to comply with all the aspects of the PPACA. Is ObamaCare using medicare dollars to help pay for this new law?
No.Medicare is an entirely separate system from the PPACA, and funded from a different source. Medicare is a social insurance program paid (primarily) from payroll taxes, and open to US citizens 65 and older.The PPACA allows for the expansion of Medicaid (medical coverage for the poor), and is funded by the states and federal government out of the general tax revenue of both.The operation of the PPACA-created Insurance Exchanges are paid for out of state funds (even if Federally-run, the state must reimburse the Federal government for proporational costs). All enforcement and compliance funding comes from the general operating budget of the relevant agencies/departments. Both of these costs should be offset by additional taxes and fees levied on the healthcare industry as part of the PPACA.The final portion, the subsidies given to the people who buy insurance through the Exchanges come from the Federal government. They are paid for from general tax revenue, which will include any PPACA-related penalties paid by non-compliant individuals and businesses.
