Yes. Franklin Pierce who was elected as a Democrat in 1852 did not get the nomination to run again in 1856. His successful Democratic successor, James Buchanan also failed to be nominated for a second term in 1856 ,but for a somewhat different reason-- the party split over slavery .

Yes. Gerald R. Ford became Richard Nixon's VP when Spiro Agnew resigned, and then moved up to President when Nixon resigned.
