There are occasions where a lack of smoking would make the movie unrealistic, such as period pieces. The best example probably is Good Night, and Good Luck. (2005). The movie is based on real events in the 1950's in which TV news anchor Edward R. Murrow is a key character. In real life, Murrow smoked while doing his broadcast! To make a movie where he did not constantly have a cigarette in his hand would be almost criminal.

Believe it or not, a lot of tobacco companies pay for movie producers to have actors smoking in their movies. It is a form of advertising called product placement.
