Sometimes your internet may not be working, and things like Java (what portions of Facebook run on) crash. Try refeshing your web browser. If that doesn't work, try emailing Facebook (their contact info can be found at the bottom of your facebook login screen; the news portion)

I believe it's because your friend has set his/her security setting to have it that only THEY can send out friend requests. No one can request them. It's too keep spammers out.
Type in the name of the person you want to send the request to in the top right corner. Select the right person on the list that will show up and click Add As Friend

Easy just look for your friend and then send them a request.
Search for them or look on another friends page who has her for a friend and add her that way
If you are referring to friend requests, that depends on the privacy settings and to let you know who accepted the friend request.
Once you find them, you can send them a friend request to add them to your profile's My Friends page. Find friends by clicking on the "Search" link. Click the "Add to Friends" link to send a friend request. If you aren't sure about the person send a short message before you send the friend request. Wait for your friend to confirm your request. When the friend accepts your request, you will be able to view her entire profile
