Yes, Roach dies. If you want to know how, read below: At the end of the mission "Loose Ends", you run with Ghost through a forest down to the designated pick-up zone assigned by General Shepherd. In the forest, Roach gets hit by a mortar round. Ghost manages to support Roach up to the helicopter, where General Shepherd promptly takes the Intel you recovered and shoots you, and Ghost. Two soldiers cover you in gasoline and then General Shepherd flicks his cigar butt at you. You see Ghost burning next to you as the helicopter flies off through the flames. Although... Ghost isn't dead, according to an interview with the game manufacturers. Not sure what they meant though.
