It is included in Vista, however Narrator is turned off by default. The Narrator exe file is located at: C:\Windows\System32\Narrator.exe.
To run Narrator at Windows startup;
1. Click Start
2. In the search bar type Narrator then press enter
3. When Narrator starts, click Preferences
4. Make sure Start Narrator Minimized is checked
