Some of the reasons that the Framers changed the Articles of Confederation are that:
It did not provide for a Executive branch on the national level.
It did not provide for a Judicial branch on the national level.
There was no national army. What there was had to be requested from the states.
General welfare was not provided for by the government.
There was no Bill of Rights.
The Founding Fathers felt the need to change the Articles of Confederation because of the desire for a stronger Government. A conflict called Shay's Rebellion created a fear of violence, and many saw stronger government as a solution. The issue of states' freedom was also a contributing factor, and how each state could be represented in the government. The initial meeting of the now-named "Constitutional Convention" was intended just to revise the Articles, but instead they ended up writing a whole new document.

One of the reasons is that the articles were made so that it was extremely hard to change them. They made a weak decentralized government to maintain the status quo, and for any change to be made to the articles you would need the approval of 9 out of 13 colonies.

The Articles of Confederation also did not have any means of resolving disputes between the states. Furthermore the Articles of Confederation did not have a strong executive branch and could not create a unified currency. Finally the Articles of Confederation did not allow for the direct participation of citizens in the government and it was difficult to amend the Articles of Confederation. The Articles of Confederation lacked the necessary powers to govern effectively leaving it unable to enforce laws collect taxes or provide for a unified military. The Articles of Confederation did not have any means of resolving disputes between the states. The Articles of Confederation did not have a strong executive branch and could not create a unified currency. The Articles of Confederation did not allow for the direct participation of citizens in the government and it was difficult to amend the Articles of Confederation.These shortcomings of the Articles of Confederation led Americans to seek a stronger more effective government that could better meet the needs of the nation.
There were many reasons that led the people to call for changes to the Articles of Confederation. One such reason was because too many states fought among themselves which in turn weakened the government instead of united the colonies against Britain.

Before our current Constitution, there was the Articles of Confederation between the various states. For a variety of reasons, the Articles of Conferation proved unsatisfactory, and so a Convention was convened to debate the formation of a "a more perfect union" through a new Constitution.
One that all 13 states had to approve it. I don't know a second one.
