Probably an air leak somewhere. Try cracking that window just a tad, sometimes helps.

The car is cold; icy cold. You are in the driver's seat, facing forward, and breathing, and your exhaled breath contains about 1% water vapor. Your breath hits the inside of the windshield. The vapor condenses on the cold windshield and freezes. If you were in the back seat or passenger seats facing sideways, those windows might also freeze. However, the windshield defroster should, within a couple of moments, clear up the windows. If you're referring to the outside windows, the windshield is tilted and water vapor can condense and freeze on it as the vapor falls. Your side windows are vertical, or nearly so, and not much water lands on it. Rear windows are tilted, but generally less than the windshield, so they don't frost up as badly.

Pour warm/hot water on the windows. It works every time! If the defroster and heating units are working correctly I don't understand why the inside windshield would still be fogged up. I have a 2000 Plymouth Breeze and the owners manual says when using the defroster be sure the the vent control knob is turned to outside air position. Hopefully this will work. NEVER!!! EVER!!! Most vehicles have settings that allow for circulated air to function as a defroster, cooler, or heater, and can be adjusted to come out by the feet, the multi-directional vents (around chest level), and those near the windshield and windows (or sometimes a combination of those locations). The only setting where the air is only blown by the windshield is defrost.
