Humans (and other mammals) have tiny air sacs in the lungs, alveoli, that are responsible for the exchange of gases to and from the blood. The same is true with fish, except their version of alveoli (gills) are external to exchange gases directly to and from the water. This allows for a process called "countercurrent exchange" by which oxygen is able to diffuse into the capillaries of the gills as the fish swims.
