It depends on a number of factors. Very often, they are homeless, so they have no place to wash their bodies nor their clothes. Maybe there are no shelters available, or they have problems which keep them out of the shelters. People with mental illnesses, violent behavior, substance abuse problems, or who are members of certain marginalized communities may have difficulty staying in shelters.
Then there are the poor people who have places to stay. They might not be able to afford transportation to go wash their clothes. Low income apartments often don't have washer and dryer access. The functioning poor might also suffer mental or physical illnesses which keep them from taking proper care of themselves.
Then there are health problems. If you have Diabetes or kidney disease, or you are up in age, you might have body odor. If you are poor, you might not be able to afford proper medical care.
