well a lot of times you don't lose it it just blows out the floor, and in the winter the heat will come out defroster vents on hills, if this is your problem there is a check valve in the vacuum line that works the vent door actuators, when under load the engine does not produce vaccum to this line so the check valve holds vacuum in the system. it should be near the intake manifold on the pass side of engine, it should only let air through it in one direction sometimes this is the problem, you could also check for a broken vacuum line and in one instance the cruise module in one of our vans was bad and leaking vaccum and created the symptom, i used to find the check valves in the "help" section at the auto parts store

Check the transfer case. If it is overfull, the rear trans seal has failed.
Could be as simple as a dirty, plugged up fuel filter or as bad as a damaged catalytic converter. If there is a check engine light illuminated, start with having the codes scanned.
transmission is either lose at the connecting point or trany has bad part inside. doubt it could be rear end. I'm thinking spark plug wires, time for a good tune up.
You will probably slam in the back of some old lady stoped at a red light. It is caused by a vacume failure, a rebuild kit is available I will try to find a link for one.

if drain plug is stopped up, u need to lose antifreeze and flush radiator and then replace with new antifreeze as obviously the radiator and coolant are dirty or drain plug would not stop up
sounds like the flywheel could be lose. does it do it all the time while driving or just shifting
