There may be a vacuum leak or a stuck EGR needle. I cleaned the EGR valve on my 87 Chevy Nova and the idle straightened out. The EGR valve was allowing exhaust gasses to escape into the fuel mix at the wrong rpm. It is suppose to open at high rpm to cool the internal chamber and prevent high temperature chemical reactions that result in unwanted emission's.
