it is probably your shift lever bushings, but check the rubber seals and carpeting, sometimes they put pressure against the shifter making it pop out of gear.
missing teeth on third gear
could be other problems like a weak retaining ball, but most likely is a simple 30 minute replacement of the shifter seat and cap that goes on end of shifter. parts cost about 30.00 at dealer and they are installed through the floor of the vehicle, no need to remove the trans. change gear oil after replacement though. can google for the problem and find many writeups on the procedure, search tacoma jumps out of gear. hope this helps. mark
