More than likely your engine temperature sending unit is bad. It is mounted on the thermostat housing below the air cleaner front (left hand side) and the upper radiator hose goes to it. It has a single slide on terminal connection and to fix it is easy. I would suggest flushing your cooling system since draining the system is required, unless a flush was done in less than 3yrs ago.
Either way drain system,remove sensor and replace(use some permatex high temp thread sealant) this is put on the threads of the sending unit to seal for leaking. Tighten hand tight. Slide connector for electrical back on, then refill coolant to proper mix 50/50, using distilled water. This is better for less corrosion and normal green antifreeze(about 1 gallon).
An easy way to ensure the gauge is OK is take the electrical plug off the sending unit and use a piece of wire and place one end in the connector and then ground to a good metal ground (the block) and turn the ignition key ON (key on only) and if gauge goes to full hot your temperature sending is BAD.
I would check this 1ST to make sure but would bet $ the sending unit is bad and this is normal with the age of the truck.Normally the gauge is fine most of the time!!!!
Coolant temperature sensor is locatedat the front of the intake manifold near the upper radiator hose.
It is located at the thermostat housing single wire plug and the upper radiator hose goes to the housing to keep it simple.
change your thermostat....

Measure the temp of the upper radiator hose very close to the warmed up engine using an infrared temp gun. If the gun reads 190 to 200 (normal) and the vehicles gauge doesn't match, replace the temp gauge sending unit on the engine. Your local auto parts store will let you use a temp gun for zip.
The temp sending unit for the temp gauge is located on the drivers side of the engine between cylinders 3 and 5 spark plugs.
