Most probably this is because the said image is not available on the server and the browser failed to load it. Another possibility is that for some site or in mails or because of a browser setting the images may not load and in these circumstances the said icon appears. In mail boxes particularly the images are blocked by default to protect your privacy.
If no image is being displayed in your browser then
In Mozilla go to tools -> options ->content-> tick load images automatically options
in IE go to tools -> internet Options -> Advanced -> Tick Show Pictures options
