There are two possibilities here:

  1. The coolant is frozen (not enough coolant was present, too much water was present, or the temperature dropped low enough to freeze the coolant).
  2. Some coolants are not meant to be mixed. I've heard it told that doing so with the wrong coolants can result in solids being formed. I've never seen it, but if this is the case, you should not run this sludge through your engine--that is unless you want to rebuild your engine.

Do you fill up the coolant reservoir to the tip top?

No , on my coolant reservoir there is a cold fill mark . When the engine is cold the coolant level is supposed to be up to the mark . When the engine is warmed up the coolant level is higher

Does coolant flow through reservoir?

No. The reservoir holds the coolant that flows back into when the engine coolant expands and will flow from the reservoir to the engine when the water cools. but there is no actual through current of liquid. Yes indeed it does on a lot of European vehicles. This occurs mainly in automatic coolant bleed vehicles.
