Just an uneducated guess, but it may be preventing his diaphragm from expanding downwards, which is how air is drawn into the lungs.

Does it hurt when you breath during a asthma attack?

It can hurt. But different people experience different symptoms. And even one person can have different kinds of asthma attacks ranging from mild coughing to painful tightness. If you want to get a sense of how it feels, take a really deep breath. As deep as you can and hold it for a few seconds. Then, without exhaling at all, try as hard as you can to inhale more (really hard, until your lungs are burning), hold it, and then do it again. But imagine that it feels that way through the entire breath, but sharp enough that I can't stop coughing. Other attacks don't feel like that at all. I don't cough much, but I'm just out of breath, tired, uncomfortable, and I can hear the wheezing but am not feeling it a lot.
