In the series, the Doctor changes his appearance periodically when the actor decides to leave. David Tennant decided to leave after 4 1/2 years to work on other projects, so a new actor took over the role. And when Matt Smith decides he wants to move on, hopefully never, another actor will be chosen. It's why Doctor Who has survived for nearly 50 years.
