Low of coolant, thermostat is stuck closed, radiator is clogged, cooling fan is not working, or blown head gasket.

If your Accord is an automatic, use the Honda automatic transmission fluid. With a manual, the Honda manual transmission fluid. I'd strongly suggest using genuine Honda fluid over a substitute.

Yes, it came from the factory with R12. R12 was used up until late 1995. I suggest you convert it over to R134.
It can fail suddenly. There is a recall on all 2003-2004 Honda Accord transmissions as there was a problem with lubrication.
Could be running backwards....reverse the leads and see if it stays cool running the opposite direction.
The second fan is usually for the when the air conditioning is turned on and only comes on when the a/c is on.
