The Commanding officer of the Pearl Harbor base and fleet lost his command due to his failure to have the sailors ready for action at all times. Roosevelt was in the middle of negotiations with a Japanese official at the time of the attack. Radio messages and warnings went unheeded so Pearl Harbor was descimated due to failures on the part of communications officers.

It has become synonymous with "surprise attack" ever since in the U.S. Unfortunately, the mistakes of intelligence collection, sharing, and analysis leading to the Japanese success at Pearl Harbor did not, in the end, lead to lessons.
