It wasn't taken away. He was awarded the medal in 2001, long after the actions that earned him the medal.
Theodore Roosevelt's Medal of Honor was not taken away. There were discussions in 1912 about revoking the honor due to concerns over whether his actions during the Battle of San Juan Hill in the Spanish-American War warranted it, but no action was taken and his medal remained intact until his death in 1919.
I believe it was Buffalo Bill Cody, who was awarded the Medal of Honor as an Indian Scout for the US Army in the late 1800's. FYI, it wasn't a "personal" thing. Hundreds of people were removed as recipients of the medal in a review done in 1917. Because the MOH was the only medal the US awarded (til the very end of WWI), many awards were felt to have been awarded carelessly, and were ordered surrendered by the recipient.
Franklin D. Roosevelt's successor in the big chair was his third Vice President, Harry S Truman, who had taken office only about three months earlier.
The proper name of the medal is simply the Medal of Honor- no Congressional. The citation for his medal read: "After his platoon had suffered heavy casualties and 3 other noncommissioned officers had become casualties, Cpl. York assumed command. Fearlessly leading 7 men, he charged with great daring a machinegun nest which was pouring deadly and incessant fire upon his platoon. In this heroic feat the machinegun nest was taken, together with 4 officers and 128 men and several guns."
