Very likely its because of wear in the shift linkage. You may be able to adjust the linkage to compensate for the wear. If not, you have a more serious transmission problem.
It may mean that you are manually "holding" the transmission from upshifting to highest gear "Holding" in 2nd not allowing to go into 3rd or "Holding" 3rd noth allowing to go into Overdrive
Your vehicle is probably in "fail safe" mode. I would have the pcm/tcm scanned.
Yes that does not sound like transmission issue to me. that is usually shown by trouble changing gears.

No, check the fluid level and have the transmission serviced. (filter and fluid replaced)
The straight bar going from gear stick to gear box is possibly bent.. Or not bolted on.. There are two bars between gear stick and the gearbox check both of them

Guessing that the power to the transmission module is lost. Some manufacturers make available a "limp home" mode that reverts to third gear take off if the electronics are lost. In other words, if you unplug the computer harness from the transmission, the transmission will remain in 3rd gear so you can drive it to a repair shop.
If it isn't slipping it is downshifting to 3rd gear
i would go for a new clutch assembly first
