Why would narcissist take out fat older women but not decent looking wife?

His behavior is not about your attractiveness or the appearance of the affair partners, but how they make him feel and the stream of attention (emotional and sexual) that they provide him. Sex is just an easy way for a narcissist to get the attention "fix" that he craves. Depending on his age, financial status and general appearance, it simply may be that your spouse is not able to have affairs with model looking women.
If he is an actual narcissist (only a professional can make a diagnosis) he's like a junkie with heroin. He continually needs an attention "fix" to feel good. To get this, he often seeks out 'fresh' sources of admiration---making conquests of women who then gush over him, flatter him and feel grateful for his meager attentions to them. He also may make a wide mental separation between his 'good' wife whom he punishes for curbing his freedom (no dinner outings, etc.) and the 'sluts' he cheats with, demeans and uses.
It's not about his cheating with these women, it's about what your husband is cheating you out of---of love, of affection, of a whole life that you deserve that you won't get as long as you stay with him. Living with a narcissist is a soul killing process that erodes your self esteem. For your own health, get away from him completely.
