We'd really need to know more about your engine swap in order to answer this. Such as, was it the same engine? If you were trying to swap in, say, a V8 where a V6 or an inline six originally set, and treat it as a "plug and play" application, then you're going to have to reprogram the ECM. Whether that's the case or not, we have no way of knowing... there were several engine options available for those years.
The 1995 engine is fully electronic, while the 1991 engine is primarily mechanical.

The only way to tell is if there are numbers wrote in chalk on the engine. then you'll know it's not the original engine for sure. but other than that you wont know.
Did you change the fuel filter? Have you checked the fuel pressure regulator?

Usually it is fatal. (for the engine) It will most likely cease up. The valves, lift and everything will be all out of time and start slamming into eachother. If it does go out then the engine wont run. it wont do much of anything.
