There is not a yes or no answer here. It is dependent upon the type of plan that you have. This is usually determined by your employer. Blue Cross Blue Shield of Alabama does cover gastric bypass surgery and you can go to their website, scroll down towards the bottom and click on providers then click on medical policies and drafts since this policy is in the process of being reviewed. You must meet the three outlined criteria. Most people with a BMI over 40 don't have a problem getting a predetermination. However, the deductible does vary according to your benefit plan.

I am considering the surgery and I called customer service on the back of my card, and if I receive a predetermination my total out of pocket expense is only $200. Whereas, my sister's BCBS policy pays only 50% of the surgery but inpatient stay is covered at 100% minus her inpatient deductible. So, I would advise you to call customer service on the back of your card and let them look at your plan and tell you what is covered and how much out of pocket costs you would be responsible for. Also, make sure that you are using one of their preferred physicians, because if not the out of network deductibles can be costly. Good luck on your decision!
