No one answered the question to my original post, so let me answer it myself. As I stated in my original question regarding if I would have difficulty obtaining a Chinese visa with a misdemeanor. The answer is, in my case, "no". I arrived at the consulate in a suit and tie, was polite, cheerful and kept my mouth shut. The reviewer looked at my description of my so-called crime, "disturbance which included raised voices with alleged pushing and shoving". His only comment was, "Your visa will be ready after 2 PM".
There are several places online that one can obtain a Chinese visa application form. Or one could go the route of visiting a Chinese embassy or any other governmental office in their area.

You can find application forms at Chinese Embassies or Consulates. In urgent situations it is possible to get a VOA (Visa on Arrival) at Entry Posts. Alternatively, some websites host the Chinese Visa Application Forms. One such site is TravelChinaGuide.
