Some models and years have interchangeable rear wheels but by no means all.

offsets are determined from center of wheel front to back if bolt holes are front of center then its front-spacing if behind center it's back-spacing measure wheel front to back and then lay wheel on front and measure to back of bolt hole that gives you offset if any.
Set back is when one front wheel is set further back than the other. With alignment equipment that measures toe by using only the front instruments, any setback will cause an uncentered steering wheel. Any good 4-wheel aligner will reference the rear wheels when setting toe in order to eliminate this problem
Any number of things can cause a steering wheel to shake back and forth. It could be a bad wheel bearing indicating a wheel is about to fall off. It could be a bad tire or bushing. Without checking it out personally, I can't tell.
The PVC wheel cover should snap onto the 4x4 rim. Look around the back side of the cover to find any cracks or missing parts, as the wheel cover should be securing itself.

Yes, there is no difference between the bearing slot in any scooter wheel. Any bearing will fit any wheel.
Most will but in certain cases it will not. Ford increased the center hole size on the rim to 2 3/4 in 1998 and above on 2 wheel drive rangers. 97 on back had 2 1/2 center holes. The bolt patterns remained the same on all Rangers and Explorers so the lug bolt pattern will fit properly regardless of year. If the rims have a 2 3/4 center hole they will fit any ranger or Explorer hub regardless of year.
