Ideally, vaginal swabs should be taken at the first visit, and sent to a lab for analysis. The lab will identify the bacteria under a microscope. They will put the swab in a petrie dish to grow a colony of the bacteria, and then they add various antibiotics to see which one kills them. Obviously, this lab work takes a few days and the doctor won't want to leave the patient untreated until the lab results come back because of the possibility of the infection migrating up the genital tract. So the doctor is likely to prescribe a good broad spectrum antibiotic like Bactrim, and it will probably work. However, if the bacteria isn't susceptible to Bactrim, the woman should return to the same doctor or to one who has her medical notes. By that time, the lab results should be back and they will have written in the report exactly which antibiotics are best for that particular infection.
