I would weigh a number of factors. Was this for a high school or a college/university? Was this a volunteer or a paid position? How long ago was this? And what sort of job are you applying for? If this was a high school volunteer position, I would include it if I was looking for work right out of high school. I wouldn't if it was after I got out of college. I would be more inclined to include it if the job involved project management and organizational skills. If it was a paid position (some colleges/universities provide scholarships or money for working on the yearbook) I would definitely include it at the first few years out of college, but after that would not be so likely to add it in.

Job application templates can be provided by your local employment placement agency. Additionally, the website www.samplewords.com has an employment application you can download for free.

you should retrieve an application for your desired employer, if not you should submit a resume
Most employers will ask the same basic information when considering you for an interview and ultimately the offer of employment. Expect to provide personal information such as your name, address, phone number, age, gender and details of your education and employment history. Also be prepared to state your availability for work and the type of work you are seeking. Besides searching online for Employment Application Templates, you can usually find an employment agency in your area that can guide you with filling out an employment application.
