For a 15 year old, the circumcision procedure is usually done under general anesthesia, so the surgery itself would be painless. The recovery can be somewhat painful if you don't take the pain medications prescribed by your doctor, but if you take them, it should only be some discomfort.
During circumcision only the foreskin is removed, the approximate size would depend on the particular patient but would be closer to 1 square inch. This does not effect the size of your penis in any way, and does not decrease the ability to have sex, or reproduce.
