There is really no way to tell. Take the car to a shop and have them replace the transmission oil filter and fluid. Make sure to tell them you want the filter replaced because some places just suck out the oil and replace it without replacing the filter.

The front seal of the tranny needs replaced, the tranny will have to come out to replace it.
It's the fluid inside the transmission that lubricates the gears and allows the transmission to function properly. It should be replaced with Diamond SPT III transmission fluid.
In any automatic transmission the fluid should be changed at the interval specified by the manufacturer. You should have the transmission oil filter replaced also at this time. Many shops will just suck out the old fluid and replace it with new without replacing the oil filter.

Yes, you have to. Engine oil, engine filter, transmission fluid, transmission filter, differential fluid, coolant have to be replaced. Also you need to check all hoses. Rotate tires, check battery. Make alignment. Probably replace sparks and spark wires. Check brake fluid and stirring fluid. Replace air filter.
Fill the transmission filter with transmission fluid before you put the new transmission filter on. You probably will not need to add any transmission fluid.
