Any answer you get on this is just going to be an educated guess. So, with that in mind a couple things for you check for is a wheel out of balance or a "bad" tire(cord separation). I Had the same experience with my Ford Sable. It would drive smooth up to 55 mph, then vibrate above that speed and smooth back out at 79 or 80 mph. I took it into Les Schaub three times having the alignment and tire balance checked with no help. I also rotated the tires but couldn't tell any difference. Finally after nearly 2 years like that I noticed an odd wear pattern on one of the tires. I replaced the tire and the vibration problem ended. I think worn ball joints and tie rod ends could also cause the vibration you describe. I know this is not really an answer to your question but rather some possible causes to check out. I'm not a mechanic so maybe you'll get some further suggestions.
sounds like a vacuum leak
Tire/wheel balance
My guess would be wheel balance.

PCV. Positive crankcase ventilation. It removes and reintroduces gasses from the crankcase back into the intake.

wiil a 1999 blazer engine fit a 2002 blazer
it could be a bad tie rod/a bad ball joint/a tire out of balance/ or it needs a front end allignment
Check the universal joints and balance of the driveshaft. When you are accelerating pressure is applied to these parts and if they are worn or out of balance or dented they will cause a vibration that will stop when you let off the gas.
