Sounds like a significant amount of water somehow got into the PLENUM CHAMBER for the AC/heater cores. There is supposed to be a drain at the bottom of the plenum [for air conditioner CONDENSATE], but it must be stopped up. There may be other possible causes, but this is the only one I can think of. j3h.
Pour warm/hot water on the windows. It works every time! If the defroster and heating units are working correctly I don't understand why the inside windshield would still be fogged up. I have a 2000 Plymouth Breeze and the owners manual says when using the defroster be sure the the vent control knob is turned to outside air position. Hopefully this will work. NEVER!!! EVER!!! EVEEEEERRRRR!!!!!! pour hot water on your cold windshield! The temperature change WILL pop your windshield.

The jet trails across the sky were caused by condensation. The condensation of the water on the outside of the glass left a ring on the table. The condensation fogged up his glasses when he walked into the house.

you keep not fogged by not wearing them:)
because of condensation
Airport fogged in; had to reroute to Dallas.
because the moisture in the air attracts to the coldness on the mirror
Mainly because you don't need a fogged up windshield if you are driving fast. Seriously, the defroster setting is where most autos go to if there is a problem with vacuum. If nothing worked in your dash controls, the one thing you would still need would be the defroster, so that is the default setting. Now, when you accelerate fast, the throttle is wide open, and there is no vacuum, or it is very low, so with low vacuum the control reverts back to defrost. It would be the same as if you un hooked a vacuum line and did not feed your dash controls, you would only have defrost.
if you smell coolant and your windows are fogged possibly it could be the heater core.
you dont either replace assembly or if covers are fogged have them buffed
