Yup i had the same problem 2 times now.The engine light came on and someting was wrong. Both times i had to get my catalytic converter fixed since my car eventually broke down because of it. after that i noticed my car ran better...till the next time i needed to replace the converter again. that or it can just be the transmission.
It's more than likely that it needs a new fuel filter, spark plugs and ignition wires. While you are in there replace the air filter also.
Sounds like you have a misfire in one of your cylinders. Change your spark plugs, check the distributor cap for cracks, and try it again.
its possible your fuel pump which is located in the gas tank could have a clogged screen or when you drive up the hill the gas level in the tank causes the fuel pump to sick up sediment in the tank.
Drive shaft is going out
It's probably a broken engine mount. I have an 04 Alero and mine was broken. Jerking stopped when I replaced it.

There are several things that can cause your Ford E2 50 Van to hesitate while driving. The most common cause is a malfunctioning ignition switch.
There can be a number of causes for this, too many to list here. If you have not had a proper tune up within the last 80K miles then I'd recommend one since one is due anyway. This can cure many issues with driveability, though it is NOT a cure all.
