Without knowing what car you have I couldn't say for sure but from my experience most cars with the indicator above the steering column it is caused by a broken nylon string that attaches from the indicator and the shift rod. These do not wear out often, most of the time it is caused by inexperienced people dropping the steering column or replacing the column. If it is not detached at the correct time in the sequence when working on a steering column it will definitely break the nylon cable.
