It can happen. For example, while normally the Secretary of State is fourth in the line of Presidential succession, when Nixon was President his Secretary of State was Henry Kissinger, who was not a "natural-born citizen of the United States" and thus ineligible for the position of President.

I don't believe any of the current cabinet members would be ineligible under this rule, but obviously that could change at any time.
