I had something similar happen a couple years ago with my cat...first the vet tested her for any urinary tract infections--she tested negative. Then they told me to make sure her liter box was not next to anything that was loud, that it should be in a safe place all by itself--and make sure its away from their food and water....However none of that seemed to work....so finally, I went out and bought her a new liter box and added liter to it--IT WORKED! The vet explained to me that maybe for some reason she may have associated her liter box with "pain"--(for example if she had a hard stool, she may have thought the liter box itself was hurting her, etc. and she did not want to use it again.) So for me, it was as simple as just getting a new liter box.
