I have seen many posts on different sites with people having the same problem.
As far as I know, it is common on 95 to 97 windstar 3.8 engines.
Most of the answers I have seen direct you to change the Intake manifold runner actuators or check all the vacuum lines.
I think most of the answers are missing the boat on one important item.
Both actuators are vacuum contolled and if you have no vacuum they will stay open causing poor drivability, poor fuel economy, and cause the service engine light to come on.
On the firewall side of the engine intake manifold there is a bracket mounting 3 solenoid valves. The one closest to the passenger side is the IMRC solenoid. It opens when engine is started and allows engine vacuum to close actuators at speeds lower than 3500RPMs.
These valves are prone to corrosion internally and will seize after a while.
You have to remove the cowl assemblies to get at the solenoid but it is fairly easy to change.
The Ford part number for the solenoid is F1HZ*9H465*A
Costs about $80.00 and will probably solve your problems.
Buy new tank of premium gas, delete code, if it returns ensure there is proper fuel pressure, then ensure intake manifold and vaccuum lines are not leaking.
Direct injection works somewhat similar to regular fuel injection, but at a MUCH higher PSI. say a non-direct injection car's fuel pressure is anywhere from 40-60SPI, a direct injection fuel pressure runs in the couple of thousands of PSI for gasoline and up to and more than 20,000 PSI for diesel direct injection.the other difference is instead of the injector spraying the fuel into the intake manifold runner (where it has to travel a short distance to the combustion chamber, some fuel doesn't stay as a fine mist, it will stick to the wall of the runner and bead up, like rain on a window) direct injection sprays, you guessed it, DIRECTLY into the combustion chamber. it gives a good boost to compression, which gives you more power and better efficiency
