I'm under the hood of mine right now and have found the exhaust manifold and the pre cat have separated. I've purchased a set of stainless headers as the Mazda cost of a new manifold was $1100.00 CAN$ That was without installation. Headers were $400 with all the piping. You'll have no more precat and one cat con will be missing but I won't cave to the dealer.

A manifold leak is usually in reference to a failure of the intake manifold gasket. It can leak air, oil or coolant. It can leak to the outside of the engine or internally into the engine. A manifold leak may also refer to the Exhaust manifold that is leaking exhaust fumes from a bad gasket or a crack in the manifold.
Depending on the location and size of the crack, you may be able to have it welded shut or you may have to replace the manifold.
Yes you CAN start your car w/o the exhaust manifold mounted.....However, if you do, you are VERY likely to crack a valve. Your exhaust system protects the valves from cool air contacting a hot valve.
If the engine knocks when it is cold, it might be a crack in the exhaust manifold, or possibly a crack in on of the exhaust manifold bolts leading to the block. The crack is small, and once warm will swell shut, thereby stopping the noise. Without repair, it might lead to valve exhaust valve damage.

Check your o2 sensors .. Could be anything between your exhaust manifold & catalytic converter, but with other cars i have owned my bank 1 o2 sensor was all i needed with the exception of 1 car that had a crack exhaust manifold.
It is not worth the effort since it will probably just crack again; cast iron does not weld well. Use this as a good excuse to put on a header. I got a nice stainless steel header for my 1991 4.0L for $185 including shipping. The exhaust manifolds are also known to crack. If you do not want a header, another option would be to watch for someone that did put on a header but is trying to get rid of the stock exhaust manifold they took off.
