Remove the pull strap by removing the cover (it slides up towards the rear window). Next, remove the Phillips screw holding the strap assembly. Use a screwdriver (cover it with a cloth so it doesn't scratch the paint) to pop out the panel from the fasteners. It seems intimidating but I just did it and it is fairly easy.
You need to remove the inside door panel, then remove the bolts that secure the mirror. It is very simple procedure.

the lower panel is held on with push on clips, 11 of them that can be "gently pried up" according to the service manual. remove the handle and strap prior to removal.

The flasher on a 99 Toyota Sienna should be beside the instrument panel fuses under the steering column. You will probably have to remove the driver's side knee bolster to get at it, which is kind of a pain.
