Lennie kills the puppy accidentally because of his strength and inability to control his actions. He later kills the girl, Curley's wife, unintentionally while trying to quiet her down as she becomes frightened by his behavior. These deaths highlight Lennie's lack of understanding and his tendency to unintentionally harm others due to his mental limitations.

She made him angry, and he was just trying to make her be quiet. (He shook her until her neck broke.) Lennie only killed 1 puppy; his own, which makes it all the more sad.
Slim told Lennie to stop stroking the puppies because he was petting them too roughly, which could harm the puppies unintentionally due to Lennie's strength. Slim wanted to prevent any harm to the animals and educate Lennie on handling them gently.

Lennie loves to pet soft things, such as mice and in this case a dress. He sees a girl with a red dress and because he likes it he reaches out to feel the dress. However, the girl becomes distressed and begins to scream. This in turn distresses Lennie and he "gets all mixed up" and he holds on because that is all that he can think to do. But the girl continues screaming and by this time Lennie is extremely scared so all he thinks to do is hold on. He is extremely strong and when George comes after hearing the girl from a distance he has to hit him over the head with a picket fence to make him let go. Upon Lennie letting go, the girl reports Lennie and says that she was raped. So people in Weed start a party to kill Lennie. Lennie didn't actually hurt the girl but he did scare her.
In John Steinbeck's novel "Of Mice and Men," Lennie does not kill George. George ultimately makes the difficult decision to shoot Lennie to prevent him from suffering a worse fate at the hands of others after an accidental tragedy Lennie causes. George kills Lennie out of a sense of compassion and mercy.
