Brake pads can be staying applied to rotor causing excesive heat build-up and warp the rotor. Usualy caused by faulty caliper or caliper slides or caliper bracket. Also possible partially collapsed brake hose to caliper and incorrect or un even wheel lug torque. And also could be caused by person cutting rotors. The best machine to cut rotors is the machine that attaches to the vehicle and cuts the rotor on the car.
