Prokaryotes have a cell wall and some prokaryotes such as gram-negative bacteria have lipopolysaccarides that are very toxic to the body causing a homeostatic imbalance/disease resulting in death. When you force contact of pathogens to other humans that have developed anti-biotic resistance this pathogen can be extremely contagious and wipe out a portion of the population.
