Ions cannot directly cross the phospholipid bilayer which makes up the main part of a cell membrane. They cross the membrane through protein channels, pores or pumps.

There are many different protein channels. Some only allow one type of ion to move through, like a sodium channel, and others exchange ions (move ions in and out at the same time, like a potassium/hydrogen ion pump).
