It could be Insertion, which means, an addition of extra DNA or RNA into a section of genetic material. Also, it could be Deletion, which means, a loss or absence of a section from nucleic acid molecules or chromosomes.

In other words for a quicker response is, Extra Chromosomes or Loss of Chromosomes can cause DNA mutations.

It can only be caused when the baby is still inside of its mother or while it is inside its egg or whatever it is in before initial birth. Some people believe in the science-fiction way of DNA altering which is also true but not to the extent of, "Hey Stan, I got freakin' super powers." Possibly to the point where you could attain a new genetic disease, but that's about it.
