What type of bond is formed between the hydroxyl group of one nucleotide and the phosphate group of an adjacent nucleotide forming the sugar-phosphate backbone of DNA?

These types of chemical bonds are called covalent bonds. Note though that the sugar-phosphate backbone does not contain nucleotides - except as like a side branch.

What are the three chemical constituents that comprise every nucleotide?

One phosphate group, five-carbon sugar and nitrogenous base are the three basic chemical constituents that every nucleotide is comprised of. They act as the subunit or monomer of nucleic acids such as RNA and DNA.
